RN Case Manager
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
LPN CC, PRN
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

STNA Certified Hospice Aide
The STNA/Hospice Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as
necessary to meet the patient’s personal needs and to promote comfort. The STNA/Hospice Aide is responsible for observing the patient, reporting these observations and documenting observations and care performed. The STNA/Hospice Aide will be assigned in a manner that promotes quality, continuity, and safety of a patient’s care.
